In order to begin any workouts or tryouts with the basketball program you must have the following forms completed, signed and uploaded into Dragonfly:

A) Physical Packet- Can complete the medical history portion electronically in Dragonfly.  You must upload the physcial exam pages showing the doctors signature, date of the physical and the

                                 cleared to participate box is checked.  

B) GHSA Concusion Form- Can sign electronically in Dragonfly

C) GHSA Sudden Cardiac Arrest Form-Can sign electronically in Dragonfly

D) DCSD Return to Participation Form-Can sign electronically in Dragonfly

E) Proof of Insurance- upload a copy of your insurance card into Dragonfly
